(mortgage statement/lease agreement, gas, water or electric bill)
Proof of Birth

TANF/SNAP Letter (if applicable)
Custody/Child Support Documents (if applicable)
What documents will I need for my child to start school?
Current School Entrance Health Form
(physical & immunization record)
